If you are a new patient to our office, please visit our patient forms page to download our new patient forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
We are in-network with most insurance plans, we also accept payments from most PPO and indemnity plans (EXCEPT MEDICAID), if you are not sure if we take your insurance or not give us a call and ask, most likely we do. We will bill your insurance on your behalf, check how much benefit is available for you to use and how much is already used so far. Even if you do not have insurance and you will pay for your care we will make it easy and affordable. Call us to make an appointment and we will do our best to answer your questions.
